Individuals must be either on active treatment, or within 5 years of treatment

Note: All mental health providers must be registered and eligible for membership with their relevant professional body and provide evidence of registration and membership eligibility to the Psycho-oncology lead in the relevant Children’s Cancer Centre for referral.
How do I apply?
Individuals and families can either self-refer or ask the hospital to facilitate a referral and use the form below to submit their application.
Hospital Mental Health Clinician/Social Worker referral
Explains how to access a Mental Health Care Plan from the family GP;
Provides contact details for 2 or 3 local psychologists/mental health provider for the family to consider;
Seeks permission to provide family contact details through the below application form to the Children’s Cancer Foundation to arrange approval for funding
Direct referral
Individual obtains a Mental Health Care Plan from their GP;
Where a non-Medicare rebate option is required, evidence of provider registration and membership is provided with the application;
The individual/family submits their details through the below application form to obtain approval for funding;
The individual/family selects their own service provider, books and attends sessions and pays for the sessions
